NAFLD, NASH, cirrhosis -- what's the difference?

Fatty liver disease is not one condition -- it's a spectrum. Where you are on that spectrum determines how urgent treatment is and what happens next.

Healthy Liver
No fat, normal function
NAFLD
Fat in liver, no inflammation. Often reversible.
NASH
Fat + inflammation + cell damage. Needs treatment.
Cirrhosis
Irreversible scarring. Transplant may be needed.

What makes NASH different from simple fatty liver?

Simple NAFLD

Fat in the liver without significant inflammation. Liver function is largely preserved. Reversible with diet, exercise, and weight loss. Most people can achieve full resolution.

NASH (Steatohepatitis)

Fat plus active inflammation and liver cell death. Requires medical management. Without intervention, 10-15% progress to cirrhosis. Cannot be reversed by lifestyle alone at advanced stages.

The only reliable way to distinguish NAFLD from NASH is a liver biopsy or FibroScan -- an ultrasound scan isn't enough. If your doctor found fatty liver on ultrasound, ask about further assessment.

Why fatty liver is often missed

Most people with fatty liver disease have no symptoms at all -- even in moderate NASH. This is what makes it dangerous. By the time symptoms appear, significant fibrosis may already have developed.

Symptoms When Present

Persistent fatigue without clear cause
Vague discomfort or heaviness in the upper right abdomen
Unexplained mild weight loss
Enlarged liver found on examination or scan
Elevated liver enzymes on routine blood tests (ALT, AST)

Signs NASH Has Progressed -- See a Specialist

Abdominal swelling (ascites) -- fluid build-up
Jaundice -- yellowing of skin or eyes
Confusion or changes in behaviour
Vomiting blood or very dark stools
Ankle or leg swelling that doesn't go away

Treatment Plan

What treatment looks like at each stage

There is currently no approved drug that cures fatty liver disease -- but lifestyle treatment is highly effective in early stages. At advanced stages, specialist-guided care is essential.

Stage 01

Simple NAFLD

Fat in the liver, no significant inflammation
  • Weight loss of 7-10% body weight -- most effective intervention, reduces liver fat significantly
  • Mediterranean-style diet: reduce refined carbohydrates, sugar, and processed foods
  • 150+ minutes of moderate exercise per week
  • Strict alcohol avoidance
  • Control blood sugar, cholesterol, and blood pressure
  • Follow-up ultrasound and liver enzymes every 6-12 months
Stage 03

NASH Cirrhosis

Irreversible scarring; advanced specialist care required
  • Management of complications: ascites, varices, encephalopathy
  • 6-monthly ultrasound + AFP for liver cancer surveillance
  • MELD score assessment for transplant eligibility
  • Living donor liver transplant -- the only curative option at this stage
  • Post-transplant: metabolic control critical to prevent recurrence
